Discharge Planning/Utilization ReviewHospitalization can be a stressful time for patients and their families, and planning for care after discharge can also create anxiety. The Discharge Planning/Utilization Review department's primary concerns are for appropriate discharge care, support with emotional situations, advocacy, and to help insure appropriate utilization of resources.
They work closely with each patient's physician to assess their needs and arrange for a plan of care which may include home referral for care, assisted living referral, county agency referrals, Swing Bed Program, financial services, and medication assistance. In addition, the department works in coordination with the patient's insurance company to obtain authorization(s) for care and information to support continuum of care.
